Grant Programs


The Arizona Health Facilities Authority has two separate grant programs. Eligible applicants for both programs must be non-profit health care organizations operating within the state of Arizona.

 

Primary Care Mini-grant Program

 

The Primary Care Mini-grant Program (PCMG) is a collaborative partnership between St. Luke's Health Initiatives, The Arizona Department of Health Services, Bureau of Health Systems Development and the Authority. Since 2005, the PCMG has awarded nearly $250,000 to more than 25 underserved Arizona communities.

 

The PCMG matches $50,000 contributions from the Authority and St. Luke's Health Initiatives. The $100,000 total available funds are awarded annually to non profit health care providers in amounts not to exceed $20,000. Seventy five percent is awarded upon selection and the remaining twenty five percent is awarded upon project completion. Eligible projects must fit within one of five broad categories: Local Primary Care Needs Assessment; Community Primary Care Strategic Planning; Community Care Improvement; Community Primary Care Identification of Alternatives and Resources; or Community Program Planning, Implementation and Evaluation. Applications are limited to 10 pages and must include both a budget and time framed goals and objectives. Successful applicants must submit a final report upon completion of their funded project in order to receive the final grant payment. The final report must include an evaluation of the outcomes of the project and a financial summary of the utilization of the grant funds. The Bureau of Health Systems Development provides applicant technical assistance and monitors the applicant's progress in implementing the PCMG.

 

PCMG applications are accepted at various times during the year. The application form and instructions will be available on this website during application cycles.

 

Small Grant Program

 

In 2009, the Authority initiated its Small Grant Program with an allocation of $150,000. Grants are available in three categories: capital equipment; technical assistance; and sponsorship of community health events and conferences. Capital equipment requests include but are not limited to waiting or exam room furnishings, computers, high speed internet access to accommodate telemedicine or electronic medical records. Technical assistance requests are limited to new construction, renovation or expansion of existing facilities and financing of these costs. Examples of eligible technical assistance requests include architectural, facility planning, financial management and fundraising. Sponsorships include health fairs and related events targeted to patients and community members as well as conferences targeted to health care professionals.

 

Capital equipment and technical assistance requests are limited to no more than $25,000 each. Sponsorship requests are limited to no more than $5,000 each. In all cases requests below these maximums are encouraged. Small Grant Program requests are accepted on an ongoing basis. To request a Small Grant please submit a letter stating:

 

  • The funding category of your request (capital equipment, technical assistance or sponsorship).
  • Description of the specific purpose of the requested funds (how the funds will be used and why they are needed).
  • Identification of any matching funds.
  • Timeline for the funded activities.
